Petition for Reinstatement
Reinstatement petitions must be complete before a decision can be rendered.
Transcripts from any schools you have attended since dismissal are required. If you do not yet have a completed official transcript, please send an unofficial transcript or copy of your current class schedule as proof of enrollment.
Transcripts/Proof of Enrollments may be sent one of the following ways:
- E-mail – Contact your university or college to issue official transcripts to our office via e-mail to vpuerec@iu.edu.
- Mail – You may mail your official transcripts to the OVPUE Records office at the following address:

Your Petition for Reinstatement cannot be initially reviewed without at least your unofficial transcripts or proof of enrollment if you have enrolled at another institution since dismissal.
